| USS Midway, CV-41 VMCJ-1 Mishap By: Dave Smith Nov. 3, 1974 USS Midway/CV-41 Aircraft #153111 (Pilot) Unknown (RSO) Lt. Paul Duncan Cause: Crashed on approach to USS Midway -Dave Smith- I remember watching Lt. Paul Duncan accompany his RF4B into the Pacific after a bolter. They never recovered him or the aircraft. He was the RIO on the plane. The pilot ejected and I can't recall his name.
